Patients Know Best shortlisted for the 2024 HSJ Partnership Awards with the Heart Failure team at Chelsea & Westminster hospital

Patients Know Best is delighted to announce that our partnership with the Heart Failure team at Chelsea & Westminster Hospital NHS Foundation Trust (C&W) has been shortlisted for HealthTech Partnership of the Year at the HSJ Partnership Awards, recognising outstanding dedication to improving healthcare and effective collaboration with the NHS. 

Following the thorough judging process, Patients Know Best (PKB) was shortlisted, ahead of the official awards ceremony to be held next year (21 March), for enabling the C&W Heart Failure team to deliver a digital first approach to supporting patients living with heart failure.  

PKB’s platform (known as Care Information Exchange in the region) has been used by the C&W Heart Failure team since 2019.  Using PKB’s interactive care plans and secure messaging, the team has been shortlisted for piloting two innovative projects to support underserved heart failure patients and timely implementation of the latest medication guidelines.  As part of the hospital’s remote monitoring programme, C&W has recorded 48% lower readmission rates on the Heart Failure virtual ward using digital tools compared to inpatient admissions for the same diagnosis.

The selected winners will be announced during the awards ceremony at Evolution London on March 21st  2024.

The awards judging panel was once again made up of a diverse range of highly influential and respected figures within the healthcare community, including; Tosca Fairchild, Chief of Staff, South East London ICB, Matthew Hopkins, Chief Executive, Mid and South Essex NHS FT, Shabina Azmi, Chief Pharmacist, Bedfordshire, Luton and Milton Keynes ICB, Professor Edward Kunonga, Director of Population Health Management, NENC ICB and NECS, as well as a range of esteemed healthcare leaders from across the UK.
